Note to all Athletes and Parents
The Tri-County Conference and the Diocese of Rockford require proof of medical insurance and a physical for every athlete.
If your athlete had a school physical for this year (as all students entering 5th grade are required), and you have provided the school with the completed physician's forms, no additional physical is required.
If your child has NOT had a school physical within the last year, a sports physical is sufficient. (A sports physical is a less detailed physical exam.)
Virtually all athletic programs for children now require a physical examination for participation. If your child has received a physical for football, dance, cheerleading, etc., a copy of that physical is acceptable, provided the exam was done within the last 12 months.
All athletes must provide a completed physical examination form prior to the first practice of the first sport they are to participate in.
